| Scheduling Your Appointments
Appointments for office visits can usually be made within a few days. For your convenience, no appointment is needed for allergy shots. Please note that there is a $25.00 charge if you fail to cancel your appointment at least 24 hours before your appointment. Please bring all your medication with you. We also provide emergent breathing treatments for those having an asthma flare. New Patient Forms You may choose to print and fill out these forms prior to your first office visit.
